How about instead of asking us, go to your states DMV site or just call up the DMV and ask.
I am not a mechanic, nor do I play one on TV. Actually my advice is probably worth slightly less than what you pay to view it.

Yes you do. You can not ride it on any public road with out a license and insurance.

yes, NY requires just a drivers license, along with insurance and a bill of sale. There is no title (except the registration is kind of counted as the title).
Scooters I have:
1985 (mostly unmodded) Honda Spree
2007 Honda Silverwing
