Key way keeps on breaking my af16 motor

What could be the reason why my key way always slips out the groove it sits in or anybody got any tips on installing a key way

Retaining nut too loose.?
Engine backfire.?
Does the Woodruff key split ?

Greetings:
Torque on the retaining nut should keep the flywheel from spinning, not the woodruff key. Are the surfaces smooth and true? Are you applying the specified torque?
